Under the supervision of the Director of Social and Community Services, the Agricultural Outreach Specialist will be responsible for providing direct outreach, case management, and health education services to ensure access to quality, enabling services, culturally competent primary and preventive health care services. In addition, the Agricultural Outreach Specialist is responsible for reducing health disparities for eligible H2A/H2B, migrant, seasonal farmworkers and their families within the CareSouth Carolina service area as well working with the local agricultural community to promote the Agricultural Farmworker Health Programs, while also building new partnerships/collaboration local farms and service providers. The duties include the following:
Make periodic visits to farms, labor camps/campsites located in CareSouth Carolina areas of service, identifying health problems and concerns ofH2A/H2B, migratory, seasonal farm workers and their families as well as other direct contacts with individuals in the agricultural farm work community. Market health care services to eligible clients, growers/crew leaders, and other farmworker advocates/ service providers
Identifies individual farmworkers and their family in need of health care services and provides referrals to CareSouth Carolina or another appropriate health care provider to ensure basic health screenings/ assessments and counseling.
Determine client eligibility for services and provide a Farmworker Health Information Card to eligible clients.
Maintain effective communication and service coordination. Perform Spanish/English interpretation/ translation services as needed.
Refer clients to appropriate health service providers and provides diligent follow-up and tracking of each service to ensure that appointments are kept and adherence to clinical directives, as needed and to ensure that individuals has achieved access and services have been received.
Assists members of the agricultural community with addressing social barriers to care and to ensure the farmworker and their families receive needed services by navigating the service systems and connecting them to the right resources; collaborates with other community organizations and programs on behalf of individuals in need of services.
Coordinates, assist with planning, and participate in agricultural farmworker health clinic and screening events as well as other community events and programs, such as health fairs, festivals, and coordinating council, advisory boards, and other community meetings.
Collect outreach data, provides activity reports, health screening data, referrals, conduct farmworker needs assessments, survey results and other information required and related to agricultural farmworker activities.
Completes Relias trainings and signs off on PowerDMS documents in a timely fashion.
Willingly assists with other duties as requested.
Monday – Friday; 8:00 a.m. – 5:00 p.m. Must be able to work after hours and on weekends.
