Community Health Worker

Job Description

The Community Health Worker provides activities and programs that improve the health and overall quality of life of CSC patients and community residents by decreasing barriers that may prevent them from accessing health and medical care; bringing educational messages about health and CSC into the community; and engaging patients and residents in improved individual health. Duties include door-to-door outreach, community/neighborhood canvassing; successful medical home and social service referral; increasing public and individual health education and awareness; the accurate reporting of information and data; positive public relations; and the integration of community services into CSC’s current delivery of care. This position is funded in whole by an external grant (Diabetes Free SC) and will require travel to multiple CareSouth Carolina centers and state travel as needed.

Education and Experience

High school diploma or GED Equivalent required.

Certification as a Community Health Worker is preferred

Experience in the health care industry preferred

Experience related to diabetes management and prevention or the care of individuals living with diabetes or other chronic diseases is plus

Hours

Full Time, 40 hours per week. This position may require occasional Saturday work.

Location

Bishopville is the "home site" - daily travel within the community and to other CSC locations is required, most often to Hartsville, Society Hill, and Cheraw.
