Job Description
The Patient Advocate works as an integral part of the clinical healthcare team. Responsibilities include but are not limited to:
- Assists patients with obtaining all available medications as prescribed by the primary care provider. This includes direct communication with the CSC Pharmacy for all aspects of pharmaceutical interactions including faxing prescriptions and distribution of delivered prescriptions.
- Completes various Prescription Assistance Program applications, voucher programs, and community resource applications and referrals.
- Collaborates with other staff to improve patient care and outcomes, continuously striving to improve the patient experience by providing high quality, patient-centered care.
- Participates in trainings, team meetings and huddles to facilitate the delivery of care.
- Collaborates with facilities outside of CSC for referral services.
Education and Experience
- High school diploma or GED Equivalent required.
- Completion a certified medical program such as MOA, CMA, or CNA preferred.
- One year medical office/community health center experience preferred.
Hours
This is a full time position, 40 hours per week, Monday – Friday 8:30 – 5:00.
