Job Description
The Reproductive Health Patient Navigator position, through support of the Fact Forward Project I.M.P.A.C.T (Implementing Meaningful Programs Across Communities Together) grant, will be responsible for providing culturally appropriate contraceptive health education, and outreach and serve as an advocate for Black and Hispanic youth and young adults ages 13-24. The duties include the following:
- Anticipate needs and potential social barriers, and proactively respond to situations as they arise.
- Assists CSC patients with needed services by addressing social barriers and connecting individuals to the right resources. To the extent possible, exhaust all available resources to assist where possible. Adequately utilize all available tools, systems, and platforms including but not limited to Thrive Hub, PointCare, eCW, Azara, FindHelp, and all other systems available.
- In collaboration with CSC care teams, conducts home visits to access the patient’s home environment, locate patients lost to follow-up, and other critical assistance.
- Actively and adequately participate in community events including but not limited to health fairs, festivals, community meetings, and other events as deemed sufficient by the director. Assist with facilitating CSC participation in community events and completing the proper paperwork for all event requests.
- Represents CSC in the community, promoting CSC and providing community understanding and education related to CSC programs and services.
- Submit all required reports on or before deadlines. Provide accurate data related to health screenings, SNAP information, ACA, Medicaid, and all other reports deemed pertinent by the department Director.
- Perform health care education either one-on-one coaching or in a group or community setting within the limits of his/her education, and/or certification. This includes techniques and tools that engage individuals in motivational interviewing, lifestyle change, self-management goals, etc.
- Assists with the development and implementation of departmental goals. Actively participates in quality improvement by conducting PDSAs and small tests of changes within their position and with other positions as a team member at CSC.

Education and Experience
- High school diploma or equivalent
- Three (3) to five (5) years of experience in the field or related field
- Ability to assess training needs and objectives.
- Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community.
- A valid driver's license is required.
- Bilingual, Spanish candidates encouraged to apply.

Monday – Friday, 40 hours weekly
