What’s the Medicare Prescription Payment Plan?

The Medicare Prescription Payment Plan is a new payment option in the prescription drug law that works with your current drug coverage to help you manage your out-of-pocket costs for drugs covered by your plan by spreading them across the calendar year (January–December).

Starting in 2025, anyone with a Medicare drug plan or Medicare health plan with drug coverage (like a Medicare Advantage Plan with drug coverage) can use this payment option.

All plans offer this payment option, and participation is voluntary.

This payment option might help you manage your monthly expenses, but it doesn’t save you money or lower your drug costs.

CareSouth Carolina Launches Enhanced Remote Patient Monitoring Program

CareSouth Carolina announces the launch of its enhanced Remote Patient Monitoring (RPM) program within the Value-Based Care Division. 

Following the successful completion of a three-year pilot program in December 2023, which saw significant improvements in patient care and clinical workflows, CareSouth Carolina is excited to introduce a more integrated and effective solution.

During the initial program, the RPM team distributed devices, provided patient education, and retrained clinical staff to improve office workflows. The culmination of these efforts has paved the way for the new iteration of the program, which began on June 3, 2024. This new phase features improved integration between monitoring devices and the electronic medical record system, eClinicalWorks, enhancing the efficiency and effectiveness of patient care.

In just the first week of the new program, the RPM team successfully recruited and enrolled 50 Medicare patients into the Hypertension RPM program. 

The team focuses on key areas such as patient engagement, medication adherence, clinical team integration, and the adoption of evidence-based practices to improve patient outcomes. Currently, the program is enrolling Medicare patients with out-of-range blood pressures, with plans to expand to other patient populations in the future.

Tyneshia Cannon, RN, RPM Team Lead, expressed her enthusiasm for the program’s potential impact: “Our goal as a Community Health Center is to educate our patients on the importance of understanding their blood pressure, the impact on their health and lifestyle, and medication adherence to yield the best results in lowering their blood pressure.”
